​What to Bring to Your Registration Appointment: 

PROOF OF YOUR CHILD'S AGE
Any one of the following constitutes acceptable documentation: state issued birth certificate (not the hospital certificate), notarized copy of birth certificate, a valid passport, or other official document that includes birth date.

PROOF OF RESIDENCY (two required) 
1. Acceptable documentation includes: a deed, lease, sales agreement, notarized statement from landlord, or mortgage information (must include name and address).

2. Current utility bill

NOTE: If residing with a district property owner, both forms for multiple occupancy must be completed and notarized (available during your registration appointment at no charge).

PARENT/GUARDIAN IDENTIFICATION
A driver’s license, non-driver identification card, passport, military identification, or Lawful Permanent Resident Card (Green Card) are acceptable forms of identification.

EDUCATIONAL RECORDS
Transcript and/or report card, IEP and any other forms relevant to your child’s education.

PROOF OF GUARDIANSHIP (if applicable)
Legal custody agreement will be placed in the student's record.


The following documents are not required for your appointment. Bringing them can expedite the registration process: 
   *Student's last report card
   *Special education documents
   *Gifted education documents 
   *504 document 
   *A custody order that you wish to provide
